Patient Appointments:  

We maintain the private practice model of medical care which means you have the opportunity to get to know and be known by YOUR doctor, midwife, or nurse practitioner.  Our providers gain a great deal of professional satisfaction in caring for you as a person, not just a number in a clinic.  Therefore, we see patients by appointment only, except in the case of an emergency.  This allows us to schedule an adequate amount of time for your visit.  Please arrive early and be sure to check in at the front desk to introduce yourself when you arrive.

If you are more than 15 minutes late, we may ask you to reschedule your appointment.  Late arrivals affect our providers as well as other patients who are scheduled after you.

Emergencies:

If you think you are having a life-threatening emergency, please call 9-1-1 to get appropriate emergency help.

If you are having an emergency that is NOT life threatening, please call our office at 602-973-3200.  If you are calling on Monday through Friday, you will be connected with our Triage Nurse for help in determining the best course of action.

NOTE:  If you are calling after-hours or on the weekend, you will be connected with our answering service and the on-call provider will return your call.

Delays: 

We understand that your time is valuable.  Therefore, we try our best to stay on schedule and minimize your waiting.  Because we provide many types of healthcare services to women, various circumstances may affect the length of time we spend with our patients.

Emergencies and baby deliveries also arise and can sometimes cause delays or changes in our office scheduling.  We will make every effort to accommodate you as close to your appointment time as possible.  Should your provider become unavailable due to an emergency or a baby delivery, we will offer you same-day care with another provider when possible.  Thank you for your understanding, and for your patience.   And, of course, when your labor begins, the same focused attention, care and presence will be offered to you.

Cancellation Policy:

If you find that you are not able to keep your scheduled appointment, please contact our office 24 hours in advance at 602-973-3200 to re-schedule and avoid a $25 no-show fee which may be charged for every missed appointment.  We understand that sometimes emergencies will interfere with schedules, but please make every effort to contact us promptly.  This will allow us to offer that time slot to another patient.  Thank you for your cooperation.

Disability/FMLA (Family Medical Leave Act) Forms:

Due to the high volume of patients requesting disability and/or FMLA, we respectfully request that all forms be submitted with the patient’s signature as early as possible.  This will ensure that we have adequate time to complete them.  There is a nominal $15 Fee for each set of forms needing to be filled outA $25 Fee is charged for each set of forms that is needed in less than 2 weeks’ time.  We will call you just as soon as your forms are ready for pick-up.

HIPAA:

Our office complies with HIPAA, the Health Information Portability & Accountability Act of 1996.  Therefore, NO INFORMATION about a patient will be released to any individual, school, business, family member or friend unless the patient or legal guardian of the patient has signed an “Authorization to Release Medical Information” form listing the individual, school, business, family member or friend as the recipient for this information.  You can download a copy of this form under the PATIENT FORMS tab.

Medical Records Requests:

PLEASE NOTE:  The Medical Record that is generated as part of your care at our practice is all we can release once you have signed an authorization; records we have obtained from other providers as part of your care here will need to be obtained from the original provider.

We require a 2-week advance notice on ALL medical record requests so that we can serve you well within the 30 day timeline allowed by HIPAA.  Once the provider has signed off on the release form, medical records will be released within 48 to 72 hours.  To ensure efficient processing of each request, we ask that you fill out the “Authorization for Release and Disclosure of Medical Records” which can be found under the PATIENT FORMS tab.

If you are not able to download and complete the “Authorization for Release and Disclosure of Medical Records,” we will accept and process your request when you provide us with the following information in letter form:

  • Current date
  • Patient’s full name
  • Date of birth
  • Patient’s address
  • Name of person or facility to receive the record
  • Address of person or facility to receive the record
  • Phone # and/or Fax # of where to send the medical record
  • Information to be released
  • Medical Record Dated from
  • Purpose of release
  • Patient’s signature
